Richard A. Qualls, Jr., D.C. has adopted the following policies
and procedures in order to protect the privacy of any individual identifiable health information that he maintains, creates
or receives.
Any and all of his employees, agents, contractors and business
associates are required to comply with the standards set forth in these policies and procedures. Failure to do so may
result in disciplinary action including termination, suspension or cessation of affiliation.
These policies and procedures address federal and state legal
requirements relating to use and disclosure of patient information including those required under regulations enacted
pursuant to the Health Insurance Portability and Accountability Act of 1996. These policies and procedures should be
applied in circumstances involving current, prospective or past patients, patients' authorized representatives and
individuals provided for by federal or state privacy rules.
